What is a All Wales Diabetes Prevention Programme
This model empowers health care support workers through education and training to deliver a face to face intervention to support people at risk of developing type 2 diabetes to reduce their risk. Participants are identified from the GP clinical systems, offered an appointment and a person-centred approach focuses on lifestyle changes. Significant reduction in progression has led to benefits to the person, staff member and wider system.
