What is a Swansea NPT 360 (homelessness) Project

Working collaboratively, the Swansea NPT 360 team have developed an integrated service to support people experiencing homelessness. Health, local authority and third sector organisations have created a digital platform for sharing information to enable person-centred support in the community. Putting the person’s needs first and building relationships has made a positive impact not just on the individual but on the teams and wider system.
