Primary Care Diabetic Foot Early Emergency Triage (D-FEET)

"We have removed the barriers so patients can access the service at any point of need. They are able to see a healthcare professional who can manage their condition without attending their GP Practice or Accident and Emergency departments. We can ensure they receive the right care at the right time by the right person the first time."

What is a Primary Care Diabetic Foot Early Emergency Triage (D-FEET)

The diabetic foot early emergency triage clinic (D-FEET) offers people with diabetes direct access to a Podiatrist with expertise in the assessment and management of their foot problems.  Focusing on each individual’s needs, they are reducing the impact of diabetic foot problems for the person, the service and the wider system.  Taking a Quality Improvement stepped approach the team have been able to scale the project with no additional funding, whilst delivering significant benefits.
