What is a Multi-Professional Homelessness Community Service
In Cardiff & Vale UHB, Occupational Therapists are leading a service working with people experiencing or at risk of homelessness. Working together to identify what is important to the individual they are identifying how different members of the multi professional team across health, social and third sector organisations can best support them to achieving their goals.
