What is a Urgent Care Practitioner (Nurse)
The Advanced Practitioner will be required to carry out clinical consultations, telephone triage and home visits on patients (within their scope of practice) contacting and attending the Central Vale unscheduled care hub.
The post holder will practice autonomously as an Advanced Practitioner within given guidelines to deliver care to specific groups of patients (within their scope of practice), contacting and attending the Central Vale unscheduled care hub.
The Advanced Practitioner will autonomously manage complex caseloads of people to reduce hospital admissions and improve clinical outcomes.
The role will involve delivering safe and effective assessment, diagnosis, treatment and/or referral of patients with primary care needs based in the Cardiff and Vale Localities.
The Advanced Practitioner will be required to undertake a clinical assessment, treatment, discharge and/or onward referral of patients (within their scope of practice) within the parameters of their training and capability and in accordance with the requirement of the Central Vale Cluster, which may be amended as required.
