Early Community Intervention for Memory Loss

"The programme consists of a really person-centred initial assessment, which has been more of a getting to know you, what really matters to you, what’s your lifestyle and how do you want that to be focused on strengths. After that, they have a number of sessions where each session is a theme. The first one is how to remember things that you need to do and each session you check back on how the one before went to see if it was successful, or if anything needs tweaking. And that’s why people like that coaching approach to try something out, checking in before moving on to the next lot of strategies. "

What is a Early Community Intervention for Memory Loss

This rehabilitation service aims to alter the experience of living with memory loss earlier in the patient journey. Through a coaching style occupational therapists support the person to learn essential strategies that help maintain their independence and support them to remain active in their communities. This service sits alongside the memory clinic and is enabling earlier diagnosis and support to those affected and their families.
