What is a Cwmtawe Cluster Mental Health & Wellbeing Model

This ‘whole system’ evolutionary approach, developed across organisational and sector boundaries, provides holistic support for patients experiencing mental health and wellbeing issues. By working with Swansea Council for Voluntary Service (SCVS), Domestic Abuse Hub, Health Board, Clinicians, and Cluster members this provision directs patients to the right person, right place, right time for appropriate mental health and wellbeing support. This has resulted in an average of 66.5% less demand on GPs and GPs feeling more confident that appropriate support is provided to their patients.
